The Photo
With regards to the photo, I need to be able to see your entire head and shoulders down to the middle of your upper arms. The photo needs to be of a high quality, please check you are in focus, you are looking at the camera and it is a minimum of 1000px wide.
Here are examples of two photos that were unusable and one that was perfect.
Unusable
Right arm chopped off at the top of the shoulder. Left arm covered by someone else's arm.
Unusable
Both arms chopped off at the top of the shoulder.
Correct
You can clearly see the whole head, both shoulders down to the middle of the upper arms. The photo is high quality, Andy is looking at the camera and in focus.
